Sports Medicine Center on OSU campus is located on the third floor of Pavilion building (three-story building) at Martha Morehouse Medical Plaza. Free parking is available in visitor parking garage. A free courtesy shuttle runs between Morehouse Medical Plaza and other campus medical buildings every 15 minutes from 7:00 AM to 5:00 PM Monday - Friday.
Please arrive about 15 minutes before your scheduled appointment time. You should first check in at Registration desk, and then go to Sports Medicine Clinic. Please bring the following items to your appointment:
- New Patient Packet

Please print and complete the forms in this packet prior to arrival. - A referral from your Primary Care Provider. Please check with your insurance company to find out if you need to obtain a referral before you can see a specialist physician.
- A prescription from your physician to your first physical therapy appointment, if you are scheduled to see a Physical Therapist.
- X-rays, notes, reports or any other documentation regarding your injury from the Emergency Department or a referring physician
- Insurance card and co-payment
- Appropriate clothing (shorts for a lower back or leg injury, tanktop or sleeveless shirt for shoulder or arm injury

In addition to providing general rehabilitation services, our downtown clinic, located at 21 East State Street, is the main site for the OSU Industrial Rehabilitation clinic and OSU Performing Arts specialty program. Conveniently located at the corner of State Street and High Street, it offers convenient hours, Monday through Friday.
